Destination Australia & New Zealand requires a signed booking form for all vacation package bookings and for every credit card transaction  This form explains the booking conditions and also acts as a credit card authorization form. This form is a Fill In PDF document - when this form has been completed you can fax this to 800 251 1257 or scan and email it to your travel consultant.

Some flight reservations require a separate Flight Only Booking form. Should this form be required, you will be notified by your travel consultant. You will also need to provide a legible copy of your credit card and drivers license together with this form.  The credit card holder must be one of the travelers and third party credit cards are not permitted.
